Notice of Privacy Practices Summary

This is a brief summary of Public Hospital District #1’s Notice of Privacy Practices. The entire Notice is available upon request. Public Hospital District #1 (PHD#1) is committed to protecting the confidentiality of your medical information and is required by law to do so. This Notice of Privacy Practices describes how we may use and disclose your protected health information to carry out treatment, payment, or health care operations, and for other purposes that are permitted or required by law. It also describes your rights to access and control your protected health information.

Public Hospital District #1 may use or disclose your protected health information as follows:

For Treatment: The hospital may disclose all or any portion of your medical record to your attending physician, consulting physician(s), nurses, technicians, medical students and other healthcare personnel who have are involved in your care. Your physician may also share your health care information with other physicians in their practice.

For Payment: The hospital may use and disclose medical information about you for the purpose of determining coverage, billing, claims management, medical data processing, and reimbursement. The information may be released to an insurance company or a third party payer or other entity (or authorized representative) involved in the payment of your medical bill.

For Health Care Operations: PHD #1 may use and disclose medical information about you in order to support quality improvement and other business activities of our organization. These uses and disclosures are necessary for hospital operations and ensure the quality of care received by our patients.

You have the following rights regarding your protected health information:

  • Right to Request Restrictions
  • Right to an Accounting of Certain Disclosures
  • Right to Request Confidential Communications
  • Right to a Paper Copy of This Notice
