A patient, or their legal representative, may inspect and obtain a copy of their medical records or have copies of medical records sent to another facility for purposes of continued care.

Newport Hospital & Health Services requires a completed and signed “Authorization for Release of Protected Health Information” form before releasing any documents. Specify what you want released and who you want it released to.

Accessing immunization records
You have several options available to access your family’s immunization information.
- The quickest, most convenient option is to sign up for MyIRMobile.com. More information is available through the Washington State Department of Health >>>
- Newport Hospital & Health Services can also accommodate your request if we have a signed Authorization for Release of Protected Health Information form designating the release of immunization records. Please allow us 2 – 5 business days to process your request.
Use MyChart to access your medical records
MyChart is Newport Hospital & Health Services’ secure, web-based patient portal available through a partnership with Providence St. Joseph Health. You can log in online from MyChart or use the MyChart mobile app to review your medical record. MyChart allows patients to view their medical record, send a message to their care team, view statements and much more. To sign up for a MyChart account, please visit MyChart.
Assigning a ‘proxy’ to access your MyChart account
If you would like to assign Authorization for Release of Protected Health Information account access to a family member (or someone you have a close relationship with), this can be done from the MyChart menu. Click ‘Sharing’ and then ‘Share My Record.’
